Tell tail

I have a 1991 225 txrp. I was flushing engine yesterday and the stream was slow to appear. It notice what appeared to be exhaust from the telltale than water started flowing. I was flushing in a barrel.
this is the first time I've seen this but have only had the boat for a couple months .
Anything I should check or just monitor?

block needs to fill up with water, so it may take just a little bit for water to come out,
but I would not think there should be exhaust coming out of the cooling water system

a leak down test of the cylinders would tell if there was a problem with a head or head gasket.
dropping the lower unit and scoping the exhaust stack might also be needed
